CAMP DUNAMIS | Pre-Registraion Form

Please complete one registration form per camper. Print and return this form with your $25 if registering before May 15, 2010 or $35 if after May 15, 2010.

After you reserve your spot, we will send you a confirmation and more detailed information.

Those entering 9th grade must send in this registration form along with a 9th Grade Special Permission Contract in order to attend camp. 9th graders will automatically be put on a waiting list until June 1 and then will be accepted in the order their paperwork was received.

Total fees will equal: $175 if registered by May 15, 2010 • $185 if registered after May 15, 2010
