Please complete one registration form per camper. Print and return this form with your $25 if registering before May 15, 2011 or $35 if after May 15, 2011.

After you reserve your spot, we will send you a confirmation and more detailed information.

Those entering 9th grade must send in this registration form along with a 9th Grade Special Permission Contract in order to attend camp. 9th graders will not be registered until all paperwork including the 9th grade contract has been received.

Total fees will equal: $175 if registered by May 15, 2010 • $185 if registered after May 15, 2011
